Eligibility
- For the Deceased Worker:
The worker must have been registered and held an active, regular membership with the MPBOCWWB at the time of death, and possessed a valid identity card. - For the Applicant:

Application Process
Offline
- Obtain the Application Form:
- Visit the Public Service Center, District Panchayat, or Urban Body Office during office hours to request the hard copy of the prescribed application form from the concerned authority.
- Fill in the Form:
- Complete all mandatory fields in the form.
- Paste a passport-size photograph (signed across, if required).
- Attach self-attested copies of all necessary documents.
- Submit the Form:
- Submit the duly filled and signed application form along with the required documents to the appropriate authority.
- Receipt & Acknowledgment:
- Request a receipt or acknowledgment from the authority, ensuring it contains the submission date, time, and a unique identification number (if applicable).
