- Agree to the declaration and click “Continue” to submit the form.
- Receive Confirmation:
- A confirmation page appears and a registration number is sent via SMS/email.
- Login:
- Log in using your username and password.
Applying for Scheme Benefits
- Obtain the Benefits Application Form:
- Visit the nearest Labour Office and collect the application form (Annexure V) or download it from the Official Website.
- Complete the Form:
- Fill in all mandatory details and attach required supporting documents (e.g., medical records, identity proof).
- Submit Application:
- Submit the completed form with supporting documents to the Assistant Director, Industrial Health of your jurisdiction.
- Review and Notification:
- The Assistant Director reviews the application and forwards it to the Office of the Labour Commissioner-cum-Welfare Commissioner, Haryana.
- Await notification regarding approval. Once approved, the monthly pension is disbursed according to the scheme guidelines.
