Application Process
Offline Process
- Collect Application Form:
- Visit the regional office during office hours and request the hard copy of the prescribed application form (Form XIII) from the staff responsible for issuing and collecting applications.
- Complete the Form:
- Fill in all mandatory fields in the application form.
- Attach self-attested copies of all required documents.
- Submit the Application:
- Submit the duly filled and signed application form along with the documents to the Assistant Director/Deputy Director at the regional office.
- Obtain Acknowledgment:
- Request a receipt or acknowledgment from the Assistant Director/Deputy Director, ensuring it includes the date, time, and a unique identification number (if applicable).

- A valid Registration Card of the Folk Artist.
- Certification of Non-Availed Assistance:
- A certificate from the Village Administrative Officer or Revenue Officer stating that the applicant has not received similar assistance from any other schemes.

- Witness Photo (from any individual who meets the legal age for marriage).
